This pain is usually worse when you raise your arm or lift something above your head. If the rotator cuff is involved, the pain is usually in the front or outside of the shoulder. The sac of fluid that cushions the tendon can also be irritated and swollen. As the tendon becomes inflamed (sore and swollen), it can become pinched between the 2 bones. This tendon is the one most often injured because of its position between the bones. Its tendon travels under the bone on the outside of the shoulder (the acromion). The supraspinatus muscle rests on top of the shoulder. This cuff keeps the shoulder steady as the arm moves. ![]() The rotator cuff is made up of 4 muscles that surround the arm bone. The joint socket is shallow, allowing a wide range of motion in the arm. The main joint in the shoulder is formed by the arm bone and the shoulder blade. Or you may not remember any specific injury, but you still feel pain in your shoulder. You might experience soreness after painting, lifting items, or playing a sport-anything that requires you to lift your arms, especially overhead. Another common cause is soreness of the subacromial bursa (a sac of fluid under the highest part of the shoulder). Pinching or impingement of the rotator cuff tendons can lead to tendinitis. When the arm is raised, the archway becomes smaller, pinches the tendons, and can cause inflammation of the tissues. The humerus (upper arm bone), certain muscle tendons, and associated bursa (fluid-filled sacs that cushion bone to prevent friction), move back and forth through a very tight archway of bone and ligament called the coracoacromial arch. As the shoulder inflammation becomes more severe, the pain will be present all the time, and it may even wake the athlete from a deep sleep. The pain occurs when the arm is lifted overhead or twisted. The pain is usually felt at the tip of the shoulder and down the upper arm. Shoulder tendinitis, bursitis, and impingement are among the most common overuse injuries of the shoulder. Overuse or wear-and-tear injuries, instability problems, strains, and sprains are common in throwing and overhead sports such as baseball, volleyball, basketball, water polo, and swimming. Shoulder dislocations commonly occur in football, basketball, and snowboarding. The athlete should contact his physician or go to the emergency room as soon as possible. A sudden traumatic injury to the shoulder, such as a fracture or dislocation, requires immediate medical attention. Shoulder pain is a common occurrence for athletes and people in general.
